Your first visit to WSM

If you are coming to us for the first time, please arrive ten minutes early to fill out paperwork for us to create a medical file for you. We will ask to see a valid Ontario Health card or equivalent for each appointment with a doctor. University students will need either an Ontario Health card or to fill out UHIP forms for university insurance coverage.

If your Ontario Health card expires, we will ask that you renew in a timely manner so our files are up to date.

If you are receiving treatment from a Massage Therapist or Pedorthist for the first time, you will be asked to fill out a brief form for their use.

If your injury is to hips, legs, knees, shins/calves ankles or feet, please bring a pair of shorts into which you may change prior to seeing any doctor, PT or RMT.

If you are being treated for back, shoulders, neck or arm injuries, please bring a Tshirt or tank top into which you may change proir to seeing any doctor, PT or RMT.

If you have an appointment with Michelle about pedorthics/foot orthotics, please consider bringing the footwear you would like fitted.
